Fill out our thorough intake forms. We encourage you to take your time. All forms don't have to be filled out at once. This process helps our practitioner put together your health story, which we will review at your Lifestyle Assessment.
During this 90 minute initial visit, you'll meet with our nurse practitioner, dig deep into your medical history, and complete a physical exam. You'll leave with a personalized 5-part health strategy to get you on the path to wellness.
Choose a Program
Our programs are designed in episodes of care utilizing our H.E.A.L philosophy. Together, with you, our practitioner will decide which program best fits your needs. Programs generally include a basic supplement allowance, advanced lab tests, and all your follow-up visits. Our Whole30 programs are a great introduction to clearing the fog of one of the biggest lifestyle impacts to our health - our diet. Our lifestyle medicine programs are deeper dives into all areas of your health issues. Together we will put the puzzle pieces back in place.

With your strategic plan, advanced testing, and coaching support along the way, we will get you on your way back to optimal health and wellness. Should you need ongoing support beyond your initial program, we have a continuation program for a monthly fee where you will have continued follow-ups and support with our practitioner.
